Adoption of Turn-Key Health’s Community-Based Palliative Illness Management™ in Arizona Enhances Care for State’s Medicare Beneficiaries

PHILADELPHIA–(BUSINESS WIRE)–#advancedillnessmanagementTurn-Key
Health,
an Advanced Illness Management (AIM) company serving
healthcare payers and at-risk providers focusing on senior
populations, announces growing adoption of Palliative Illness ManagementTM –
PIMTM. A comprehensive network of palliative care
practitioners, now spanning 76 percent of Medicare beneficiaries covered
by health plans in Maricopa, Pima counties, Arizona, utilizes local
resources complemented by predictive analytics to identify individuals
earlier in the disease trajectory, and support plan member and family
caregiver engagement.

“PIMTM identifies seniors enrolled in health plans
throughout Arizona who have a life-limiting illness and may experience
an over-medicalized or inappropriate death,” says Greer Myers,
president, Turn-Key Health.

He explains that over-medicalized death includes: chemotherapy for
cancer patients within 14 days of death, unplanned hospitalization
within 30 days of death, more than one emergency department visit within
30 days of death, hospital admissions within 30 days of death and
life-sustaining treatment within 30 days of death.

“Our local community-based palliative care partners are specially
trained to conduct personal, in-home patient assessments and help
individuals and their caregivers to set personalized goals of care. As a
result, plan members avoid inappropriate, costly and often unwanted
interventions and care.”

As of 2015, in Arizona, people
over 65 comprised 16.4 percent of the population — with 14.2 percent in
Maricopa County, and 18.4 percent in Pima County. By 2020, one in four
residents will be age 60+. Arizona’s Medicare beneficiaries represent a 17-percent
share of total population. According to the most recent Census Bureau estimates, the
state’s retirement-age population grew from about 25 percent of
residents in 2010 to 27 percent of the overall population in 2013.

“Payers in Arizona are enthusiastic about PIM™ because it concentrates
on quality care and is proven to reduce the overall economic burden
for plan members, as well as health plans,” says Myers.
