SCAN Health Plan Executives to Address AHIP’s National Conference on Medicare and Medicaid in Washington, D.C.

LONG BEACH, Calif.–(BUSINESS WIRE)–Three SCAN Health Plan executives will be among the faculty at the
National Conference on Medicare and Medicaid October 19-22 in
Washington, D.C. The conference is sponsored by America’s Health
Insurance Plans (AHIP), the national political
advocacy
and trade
association
comprised of approximately 1,300 member organizations
that provide health
insurance
to more than 200 million Americans.

On October 19, SCAN’s vice president of pharmacy services, Sharon
Jhawar, will discuss SCAN’s pharmacy quality solution program, which
encourages, engages and rewards pharmacists for taking a more active
role in the health of seniors. Evidence has shown that an enhanced
patient/pharmacist relationship can lead to a better patient experience,
improved medication safety and improved quality of care, particularly
for those Medicare beneficiaries with diabetes, high blood pressure or
high cholesterol. Joining Jhawar in the presentation will be Snezana
Mahon, senior director of Medicare for Express Scripts, SCAN’s partner
in this unique program.

Lisa Roth, SCAN’s vice president of care coordination, will address the
conference on October 20 discussing key considerations for health plans
who are working to develop health-risk assessments that efficiently and
effectively evaluate the physical, psychosocial and functional needs of
vulnerable populations. Her focus will be on Special Need Plans (SNP)
and health-risk assessments for those dually eligible for Medicare and
Medicaid. At SCAN, Roth is responsible for special needs plans’
activities including verification of eligibility, health risk
assessments, care plans, care management, and care coordination,
including long-term services and support for dually enrolled members. In
addition, Roth is responsible for the oversight of the SCAN PAL, a
specialized customer service and care coordination unit designed
specifically for dually enrolled members, as well as the SCAN Buddy
program, a member-outreach program to address quality, in accordance
with national quality measures.

At AHIP’s Dual-Eligible Summit running concurrently, SCAN’s Senior Vice
President of Government and Community Affairs, Peter Begans, will
discuss the future of integrated care programs. In his role at SCAN,
Begans has been active on the national stage advocating for the quality
improvements and cost-saving benefits associated with integrated care.
Repeated studies have shown that seniors enrolled in integrated care
experience fewer hospital admissions and readmissions, fewer emergency
room visits, and lower overall medical costs.

That is because such care emphasizes prevention, early intervention, and
effective care coordination including geriatric health-management or
disease-management programs, medication-management programs, and
appropriate utilization of services.
