Health Partners Medicare Expands to Lancaster, Lehigh and Northampton Counties

Offering innovative, unique plans as part of a new wave of expansion
to increase access to quality health care for area residents

PHILADELPHIA–(BUSINESS WIRE)–Nationally recognized for its innovative approach to health care,
Pennsylvania-based, managed care organization Health Partners Plans
(HPP) will offer its popular Medicare Advantage plans to residents of
Lancaster, Lehigh and Northampton counties for coverage effective
January 1, 2017. The Centers for Medicare and Medicaid Services approved
the expansion for the not-for-profit health care organization,
broadening its reach beyond Bucks, Chester, Delaware, Montgomery and
Philadelphia counties. The expansion will allow more than 237,000
Medicare beneficiaries the option to choose Health Partners Medicare for
their health care coverage.

HPP is a nimble, innovative health plan recognized for its novel
programs that improve member health while addressing its social
determinants. Most recently, the health care organization formed a
groundbreaking partnership with a Pennsylvania-based, nonprofit food
access program, Metropolitan Area Neighborhood Nutrition Alliance
(MANNA), to address the lack of affordable, nutritious foods for its
most chronically ill members. The first-of-its-kind collaboration
provides three medically nutritious meals a day, seven days a week, to
HPP members with chronic illnesses, a benefit that is included in its
Health Partners Medicare Special plan for up to four weeks. The food is
prepared and distributed to members’ doorsteps — all at no cost to them.
HPP also provides vital nutrition counseling and comprehensive care
management to teach members how to better manage their illness
independently.

Starting October 15, Health Partners Medicare will begin accepting
enrollments in the region for 2017. Health Partners Medicare offers
comprehensive plans to fit any health need and budget. Each of the three
plans offered in the expansion counties are designed to give members all
the benefits of Original Medicare while providing the high-quality,
novel care Health Partners Plans is known for.

  • Health Partners Medicare Value (HMO) – a $0 premium plan with
    prescription drug coverage.
  • Health Partners Medicare Prime (HMO) – an enhanced plan providing
    extra benefits and prescription drug coverage with a low premium
  • Health Partners Medicare Special (HMO SNP) – our popular Special Needs
    Plan, including prescription drug coverage, open to anyone who has
    both Medicare and Medicaid

Each plan includes $0 or low-cost PCP visits, an extensive network of
doctors and leading area hospitals, specialist visits with no referrals,
friendly 24/7 phone support, unlimited days for inpatient hospital stays
and fitness center and Weight Watchers® memberships. Plans
also include access to an innovative service that connects plan members
with doctors by phone or video for help with non-emergency medical
issues.
